Under our Fee for Service plan, patients pay for care each and every time they visit the office. Your healthcare will be managed like a traditional office such that contact with providers is generally limited to scheduled appointment times. Our office has contracted to offer reduced lab fees and radiology fees at participating locations. We are committed to offering excellent healthcare while saving our patients money!

A time to check on patient’s progress since last appointment. Appropriate follow up can help identifying misunderstandings, answer questions, make further assessments and adjust treatments. Patient should write down questions, medications from other providers, if appropriate, outpatient blood, pressures, and or blood sugars. Price based on complexity.
This type of appointment is for NEW symptom or a recent change in your health that needs evaluated as soon as possible. Examples include: fever, sore throat, cough, vomiting, diarrhea, laceration. These visits may vary from simple to complex depending on severity.
This type of appointment is for a new health problem that is not urgent. For example, knee pain that has worsened, a rash that has persisted for several weeks or indigestion not controlled with OTC measures.
This exam is typically done yearly and is a time to provide routine preventative care. We may review overall health (including risk factors, such as exercise, diet, smoking, and environmental, exposures) and order any age specific screening tests and immunizations. The goal is to identify early risk factors or early signs and symptoms of chronic disease, and counsel on how you can reduce risk and improve overall health and risk factors. Blood work may or may not be a part of this visit.
These visits are scheduled at specific times throughout a child’s life. They give the provider a chance to make sure your child is developing appropriately, receive recommended vaccinations, evaluate vision, hearing, height, and weight. This is also a time for parents to ask questions about their child.
A GYN visit or Well Woman Visit may include a clinical breast, exam, an exam of vulva, vagina, and cervix. A swab of your cervix may be taken, if age appropriate. An internal or pelvic exam is also included, if appropriate. Questions or concerns may be addressed regarding menstrual cramps, sexual dysfunction, birth control, or menstrual issues. Includes PAP Lab fee.
This would include repeat swab if PAP is abnormal. No other exam is included unless lab is indicated.
Initial visit, (does not include labs). Phone calls will be billed based on time.
The decision to get an IV is made at a visit prior to actual IV. The IV visit is just that -only the infusion is administered. This does not include a visit with any provider.
If on-call prescription called into a pharmacy a charge will be sent to the patient on the next business day.
This is done case by case basis. Usually homebound and or elderly patients.
These are rare visits as most are referred to Urgent Care or ER if a visit is needed outside of office hours.
